Lincoln Mushroom Farm (1975) Inc. is referred to as Lincoln Mushroom Farm Ltd or as Lincoln Mushrooms or as ‘LMF’ hereafter. Lincoln Mushrooms require employees living in employer-provided housing to complete this screening every day.

This screening is mandatory and must be completed using the secure link provided by the company. The link works on all mobile phones, tablets, and computers.

All employees, including Temporary Agency Workers and Temporary Foreign Workers (TFWs) offsite are recommended to complete the daily online self-screening assessment before entering any worksite.

It is vital for ‘early detection’ to reduce the risk of spreading infectious diseases so please let’s work together and keep us healthy and safe!

Daily Screening Requirement

1. Complete the online screening before arriving at work.
2. Email your screening confirmation to bookkeeper@lincolnmushrooms.com.
3. Report any symptoms immediately, isolate, stay home and contact Supervisor, HR, and Occupational Health.

When to Seek Medical Attention

Call 911 immediately if you are experiencing emergency symptoms such as severe difficulty breathing, chest pain, loss of consciousness, or confusion.

If symptoms worsen, notify our nurse and Occupational Health:

Screening Questions

Do you currently have any of the following symptoms?

• Fever or chills
• Cough or barking cough
• Shortness of breath
• Loss of smell or taste
• Muscle or joint pain
• Extreme fatigue
• Sore throat
• Runny or congested nose
• Headache
• Nausea, vomiting, or diarrhea
• Abdominal pain
• Pink eye

OR
Have you been instructed to isolate by a medical or government authority?

OR
Have you tested positive in the past 10 days?

Guidance Based on Your Answers

• If you do NOT pass: Do not come to work. Follow isolation rules and notify HR & Occupational Health.
• If you pass: You may attend work and monitor your health throughout the day.
